1) Two witnesses (no notary):  POA Form 1
Alberta, British Columbia, New Brunswick,  New Foundland and Labrador, NORTHWEST TERRITORIES, Nova Scotia,  Nunavut, Ontario, Prince Edward Island,  Quebec, Saskatchewan

 

2) Notary and One Witness: POA form 2
MANITOBA

 

3) Witnessed by a lawyer who is not a spouse or relative and accompanied by a certificate of legal advice: POA Form 3
YUKON TERRITORY
 

Notary must not be a spouse, partner or family member.

  • Witness must not be a spouse, partner or family member.
  • All parties must be present at the same time for signing.
  • Even though no notary is required in most provinces, you may wish to ensure the validity of the document by signing in the presence of a Notary Public as well as witnesses.
